Flagyl is prescribed to treat a variety of infections. It belongs to a class
of antibiotics known as nitroimidazoles. It works by stopping the growth of
bacteria and protozoa.
Flagyl is also prescribed alone or in combination with other antibiotics in treating abscesses in
the liver, pelvis, abdomen and brain caused by susceptible anaerobic bacteria.
Flagyl topical gel is prescribed for treating acne rosacea, and
the vaginal gel is prescribed for treating bacterial vaginosis.
Flagyl only treats bacterial and protozoal infections. It will not
work for viral infections (e.g., common cold, flu). Unnecessary use or overuse
of any antibiotic can lead to its decreased effectiveness.
Flagyl is also prescribed in treating infection of the colon caused by a
bacterium called C. difficile. (Many commonly-used antibiotics can alter the type of bacteria
that inhabit the colon. C. difficile is an anaerobic bacterium that can infect the
colon when the normal types of bacteria in the colon are inhibited by common
antibiotics. This leads to inflammation of the colon (pseudomembranous colitis) with severe
diarrhea and abdominal pain.)
Symptomatic Trichomoniasis - Flagyl is indicated for the treatment of symptomatic trichomoniasis in females and males when the presence of thetrichomonad has been confirmed by appropriate laboratory procedures (wet smears and/or cultures).
One-day treatment: 2 grams of Flagyl, taken as a single dose or divided into 2 doses (1 gram each) taken in the same day.
Seven-day course of treatment: 250 milligrams 3 times daily for 7 consecutive days.
Asymptomatic Trichomoniasis - Flagyl is indicated in the treatment of asymptomatic females when the organism is associated with endocervi-citis, cervicitis, or cervical erosion. Since there is evidence that presence of the trichomonad can interfere with accurate assessment of abnormal cytological smears, additional smears should be performed after eradication of the parasite.
One-day treatment: 2 grams of Flagyl, taken as a single dose or divided into 2 doses (1 gram each) taken in the same day.
Seven-day course of treatment: 250 milligrams 3 times daily for 7 consecutive days.
Treatment of Asymptomatic Consorts. - T. vagi-nalis infection is a venereal disease. Therefore, asymptomatic sexual partners of treated patients should be treated simultaneously if the organism has been found to be present, in order to prevent reinfection of the partner. The decision as to whether to treat an asymptomatic male partner who has a negative culture or one for whom no culture has been attempted is an individual one. In making this decision, it should be noted that there is evidence that a woman may become reinfected if her consort is not treated. Also, since there can be considerable difficulty in isolating the organism from the asymptomatic male carrier, negative smears and cultures cannot be relied upon in this regard. In any event, the consort should be treated with Flagyl in cases of reinfection.
Amebiasis - Flagyl is indicated in the treatment of acute intestinal amebiasis (amebic dysen-tery)and amebic liver abscess. In amebic liver abscess, Flagyl therapy does not obviate the need for aspiration or drainage of pus.
The usual dose is 750 milligrams taken by mouth 3 times daily for 5 to 10 days.
Anaerobic Bacterial Infections - Flagyl is indicated in the treatment of serious infections caused by susceptible anaerobic bacteria. Indicated surgical procedures should be performed in conjunction with Flagyl therapy. In a mixed aerobic and anaerobic infection, antimicrobials appropriate for the treatment of the aerobic infection should be prescribed in addition to Flagyl. In the treatment of most serious anaerobic infections, Flagyl I.V. (Flagyl hydrochloride) or Flagyl I.V. RTUR (Flagyl) is usually administered initially. This may be followed by oral ther-apy with Flagyl (Flagyl) at the discretion of the physician.
The usual adult oral dosage is 7.5 milligrams per 2.2 pounds of body weight every 6 hours.
INTRA-ABDOMINAL INFECTIONS, including:
- Intra-abdominal abscess;
- Liver abscess;
The usual dose is 500 milligrams or 750 milligrams taken by mouth 3 times daily for 5 to 10 days.

BONE AND JOINT INFECTIONS, as adjunctive therapy, caused by Bacteroides species including: the B. fragilis group.
ENDOCARDITIS caused by Bacteroides species including the B.fragilis group. To reduce the development of drug-resistant bacteria and maintain the effectiveness of Flagyl and other antibacterial drugs, Flagyl should be prescribed only to treat or prevent infections that are proven or strongly suspected to be caused by susceptible bacteria. When culture and susceptibility information are available, they should be considered in selecting or modifying antibacterial therapy. In the absence of such data, local epidemiology and susceptibility patterns may contribute to the empiric selection of therapy.
LOWER RESPIRATORY TRACT INFECTIONS, including pneumonia, empyema, and lung abscess, caused by Bacteroides species including the B. fragilis group.
Treatment:
- Acute ulcerative gingivitis: 200 mg three times daily for 3 days (Children 3-12 yrs: 100 mg three times daily; 1-3 yrs: 50 mg three times daily);
- Amebic dysentery: 750 mg orally 3 times daily for 5-10 days;
- Amebic liver abscess: 500-750 mg orally three times daily for 5-10 days;
- Amoebiasis: Invasive intestinal disease in susceptible patients: 800 mg three times daily for 5 days (Children 7-12 yrs: 400 mg three times daily; 1-7 yrs 200 mg three times daily);
- Amoebic liver abscess extra-intestinal amoebiasis: 400-800 mg three times daily for 5-10 days (Children 7-12 yrs: 200-400 mg three times daily; 1-7 yrs: 100-200 mg three times daily);
- Anaerobic infections: 1 g suppository every 8 hours for 3 days (Children 5-12 yrs: 500 mg);
- Anaerobic infections: 400 mg three times daily for 7 days (Children 7-12 yrs: 200 mg three times daily; 3-7 yrs: 100 mg three times daily; 1-3 yrs: 100 mg three times daily);
- Anaerobic infections: 75 mg/kg orally every 6 hours not to exceed 4 grams daily;
- Appendicectomy: 1 g suppository (At diagnosis/before surgery and every 8 hours for 48 hours after surgery) (Children 5-12 yrs: 500 mg);
- Asymptomatic cyst passes: 400-800 mg three times daily for 5-10 days (Children 7-12 yrs: 200-400 mg three times daily; 1-7 yrs: 100-200 mg three times daily);

- Elderly patients: Pharmacokinetics may be disturbed therefore monitoring of serum levels should guide dose adjustment;
- Elective colonic surgery: 1 g suppository every 8 hours for 48 hours before AND after surgery (Children 5-12 yrs: 500 mg);
- Giardia: 250 mg orally three times daily for 5 days;
- Giardiasis: 2 g once daily for 3 days (Children 7-12 yrs: 1 g once daily; 3-7 yrs: 600 mg; 1-3 yrs: 400 mg);
- Helicobacter pylori: 800-1500 mg orally daily for several days in combination with other drugs;
- Hepatic Disease: Dosages should be decreased in patients with severe hepatic disease;
- Intestinal disease in less susceptible patients; chronic amoebic hepatitis: 400 mg three times daily for 5-10 days (Children 7-12 yrs: 200 mg three times daily; 1-7 yrs: 100 mg three times daily);
- Pelvic inflammatory disease (PID): 500 mg orally twice daily for 14 days in combination with other drugs;
- Rosacea: apply topical gel 075-1% once daily;
